Hi!
Depends only on the steering bar you use. There are many different ones
available - made from aluminum, colored, stainless steel. So can try your
sitting position you like and let a friend measure the heigt and the
distance to the base. Then a look in the catalogue and - voilá - take the
right model for you.

I use an aluminum model in blue which heightens up 90mm (around 3 1/2 inch)
and is around 100mm (around 3 15/16 inch) nearer to the driver. All possible
things like the extension for the brake hose, extensions for the clutch
cables a.s.o. are part of the complete set. The set costs without steering
bar 199 Euro (est. US$ 223) - the bar costs around 46 Euro (est. US$ 40).

There is an importer in the states:

Spiegler Brake Systems USA, LLC
96C Westpark Road
Dayton, OH-45459, USA
Tel.: 001-937-291-1735
Fax: 001-937-291-2368

Maybe the guys there could give you exact prices.

BTW: I have nothing in common with LSL - I will get no fees from there. I am
a happy customer because I got what I wanted for my bike...
...and I apologize for my English - it's definitely not my native
language... :-)
