Good lord...I'm sorry to hear about that.  Must have caused quite a bit of
excitement for anyone following you.

How did it come off?  Did the mounting bracket come loose, or was it just
user error (not locking it down)?

Ever since I found my mounting bracket to be loose, I've been checking them
every thousand miles or so.
