This will depend on how much of the service you have them do. You can
save by changing your own fluids (brake, oil, coolant). Check with
previous owner to see what has been done when, like fork oil as that
should be done ever 12k/2yrs and to me is a hassle I still let the shop
do. The other biggies are the valve clearance check and the throttle
bodie sync. I've seen 12k services from $600-800 depending on needs and
hourly rates so doing what you can yourself really helps.

I'm sure you'll hear about Jack Lilley in the UK which is the liST
sponsor, and others like Great Bay, Arizona Motorsports, Hermy's, etc.

Ya' know, I don't recall the plate coming with the adaptor kit as I
already had a spare one about that I used, and I can't imagine for that
price it would include it. But in my notes I see I have partno A9500012
priced at 52.50 for the adaptor kit, and partno A9500011 at $2.00 for the
top plate so guessing from that they are separate. And if you saw what
the adaptors were, a couple of bolts and a couple of "T" nut thingies
(hard to describe!) I bet you could come up with something to marry the
plates at a hardware store for a whole lot less.

Anyway, sorry so long winded, just wanted to say welcome!
