I had a rear axle sieze up and lock the back wheel on a Honda CB750 K6
way back in 79 IIRC. I also had a kick starter spring break on a T500
Suzuki in 77 resulting in the kick starter dropping down, engaging and
locking the back wheel. I had a massive rear tyre blowout on a CX500
Eurosport in 83 while I was negotiating a roundabout. That was fun.

In all cases I didn't come off. The trick with rear wheel lock ups is to
break as hard as one can. The rear wheel will lift off the deck enough
to keep it stable. At high speed say 100mph the bike will stay stable.
It's not until you get down to lower speeds that the back end starts
waging around like doggydoo's tail...

Just my 10 cents.
