They don't all do that.  I don't personally remember anyone talking 
about coolant in the oil, though I'm sure it's happened to someone at 
some point and my memory isn't all that good..

The head does come out the top of the frame, at least on the ST.  I 
believe the ST and the RS frame are identical.   It's a tight fit, but 
it comes out.  Triumph sells a top end gasket kit for reinstalling the 
head and you will need some special tools and a  shop manual if you 
decide to do it yourself.

The triple is a sleeved engine and the coolant surrounds the sleeves.  
It's entirely possible that the leak is going past one of the sleeve 
seals which are essentially some sort of caulk-like sealant.  The 
sleeves can be pulled from the top of the case once the head is off 
without removing the pistons.

If you have the head off, have it checked for flatness.  It's a lot 
easier at that point than tearing it down again.  If you have a high 
mileage engine, take the opportunity to check out the valves and seats too.
