An ent doctor? Are we talking about those tree dudes from "The Lord of
The Rings?" Where I come from we call those doctors "landscapers."

Anyway. Like fingerprints, everybody has different ear canals. What
works well for one fails miserably for another. For my daily commute I
use Howard Leight Max disposable ear plugs. I know that people with
narrow ear canals find them painful. For me they are perfect. Yet the
Howard Leight QB1 reusable banded earplugs are somewhat uncomfortable
for me.

For audio earphones on those longer trips I use a set of Etymotic
Research ER-4P. I must have "average" ear canals because the stock
silicone plugs are comfortable. They block out external noise quite well
and sound fantastic. Like David Earl suggested, the spit trick makes
insertion go smoothly.
