FWIW, any audiologist can make an ear mold, which can then be sent to any number of companies to produce a custom in-ear monitor. I got mine from EAR, Inc. in Boulder. They have great service and have replaced misbehaving earpieces twice in the last two years without so much as a question. They can make your molds with any number of transducers, from ~$125 to well over $500.
IMHO, it's a lot better to go to a local guy than someone who travels with trade shows or motorcycle rallies. If you have issues, it's a simple trip in the car vs. phone calls and shipping.
