All our gas here in the corn belt contains "up to 10% Ethanol." So I'm
screwed. I've been running premium (93 R+M/2)...
My mileage figures around 40 mpg (US).

By EPA mandate, we have 10% ethanol blend in the winter months and my
mileage is usually almost exactly 10% lower even when the weather is
unseasonably warm like last winter. If I fill up outside the metro
area where 'real' gas is available, I notice a performance improvement
right away.

No, the recommended octane coverts to 89 or mid-grade at most US
pumps.
