[ST] Rear brake pads knackered!

I noticed a grindy feeling coming from the rear on my ride home and discovered that the left rear pad was gone - no brake material left on the backing just the backing grinding lovingly into the rotor. Just replaced the rear pads in early spring so they couldn't have had more than 3-4K on them. Now my rotor is shot. About $200 for a replacement.
Pads we Galfer from Dennis Kirk. I suspect the glue on the pad gave way or they were dragging and I didn't notice. Anyone else have this happen?
